Found it in Frozen Valley, south of Pilgrim's Crossing, at first I thought it was just a stray elemental from a rift or something. Followed it uphill from 4205, 1630 to 4100, 1580, which is a fair distance from the other spawn points, so a /tar Swirling Tempest macro would probably be very useful in finding this one. Don't know its full route there since another player killed it when it stopped. /setwaypoint 3698 1265 /setwaypoint 4000 1805 /setwaypoint 3885 1444 /setwaypoint 4205 1630 /setwaypoint 4100 1580

If you are playing a soul without strong instant-cast spells, you will not be able to beat this thing, as it spams stuns. It leaves a fraction of a second between stuns, so if you hammer your keybinds for instant abilities as fast as you can, you MAY get a cast off between stuns. Maybe. They better be high-DPCT dots, shields, and heals, too, or you will simply be unable to do anything for upwards of 30-60 seconds while it slowly kills you. And it drops garbage items anyway.
